SUMMARY STATEMENT:
The GRYD FCM Case Manager is a community-based position focused on outreach, case management, and client advocacy for gang affiliated/involved youth ages 14 to 26. Under the supervision of the GRYD FCM Program Manager, the case manager will be responsible for participant recruitment, linking clients to extended services, developing individualized care plans, implementing diversion activities, and advocating for clients in various settings, including court. The role requires a high degree of independence, self-motivation, and the ability to engage with participants from diverse backgrounds in their communities.
DUTIES AND RESPONSIBILITIES:
Case Management:
- Caseload: Manage a caseload of 20 participants, providing individualized support and case management, while ensuring the timely completion of all required documentation.
- Building Relationships: Contact service providers and school personnel in contracted areas to provide information about GRYD services and encourage participation.
- Advocacy and System Navigation: Provide advocacy services for participants, including attending probation hearings, school meetings, court dates, and DCFS meetings.
- Community Engagement: Participate in the Summer Night Lights (SNL) program, which runs throughout the summer. SNL is a key component of the GRYD Comprehensive Strategy, designed to provide positive alternatives in communities impacted by gang violence.
- Documentation: Maintain accurate and organized participant files, documenting activities, progress, and other contract-mandated information, including using multiple case management data tracking systems.
- Reporting: Share program data through regular meetings, check-ins, and assessments.
- Care Plans & Assessments: Coordinate supportive services for clients, ensuring comprehensive and timely follow-up. Provide referrals for youth and families to extended community-based services and maintain strategies for ongoing resource connections. Conduct extensive client assessments for the purpose of setting goals and tracking progress.
- Team Participation: Actively contributing to team-building activities within the CIS and GRYD programs, as well as participating in required events during both weekdays and weekends. Represent GRYD at community events such as health fairs, forums, and other public functions to increase awareness and engagement.
- Transportation: Be willing to transport clients and use personal vehicle for work purposes.
C. QUALIFICATIONS & EXPERIENCE:
- Education and Experience: Bachelor's degree in a related field, or equivalent experience in case management, social work, or a similar role.
- Legal Eligibility: Permanent resident or US citizen with proof of work eligibility.
- A criminal background check is required for work with children and adolescents.
- Driver’s License and Insurance: Must have a valid California Class C driver's license, minimum automobile insurance, reliable transportation, and be willing to transport clients and use a personal vehicle for work purposes.
- Adaptability and Professionalism: Ability to work independently in a community-based environment, while managing a caseload and meeting program requirements.
- Proficient in Computer and Data Systems: Experienced with utilizing case management software to track and document client interactions, progress, and outcomes. Comfortable with basic computer operations, including data entry and general software navigation.
- Bilingual in Spanish is preferred.
- Successful completion of a TB test upon hire required.
